Hoy continuaré con un segundo capítulo de mi dias de guardía, esta vez la guardia ocurrió el día jueves 20/05/2021. Como te comenté en el primer capítulo, las guardias corresponden desde la 7 de la mañana hasta la 7 de la mañana del otro día, a veces se puede extender más el horario, es decir 24 horas de trabajo.

En este lapso de tiempo siempre nos llegan muchos casos, los que predominan son embarazadas con diversos riesgos por lo cual se tienen que someter a cirugías, también podemos ver pacientes con apendicitis, obstrucciones intestinales, fracturas y entre muchas más patologías que necesitan resolverse en el quirófano, siendo algunas mas complicadas que otras.

Today I will continue with a second chapter of my duty days, this time the work shift took place on Thursday 20/05/2021. As I told you in the first chapter, the shifts go from 7 am to 7 am the next day, sometimes the schedule can be extended more, that is 24 hours of work.

In this period of time we always receive many cases, the predominant ones are pregnant women with various risks and therefore they have to undergo surgery, we can also see patients with appendicitis, intestinal obstructions, fractures and many more pathologies that need to be resolved in the operating room, some being more complicated than others.

Para esta guardia, tuvimos 14 pacientes, de los cuales 3 eran niños de 8 meses, 2 años y 4 años de edad. No pude obtener el consentimiento de los padres para tomar fotos y poder publicar los casos, y es que cuando son muy pequeños y el representante no esta junto a ellos se hace difícil de que acepten que se les tome foto y es totalmente entendible, siempre hay que cuidar el pudor y vulnerabilidad de los pacientes.

También en el día tuvimos pacientes embarazadas, pero lo intrigante de la guardia, fueron dos casos que se ven muy poco en quirófano, y es que estos casos agarraron de sorpresa a todos.

For this work shift, we had 14 patients, of which 3 were children of 8 months, 2 years and 4 years of age. I could not get the consent of the parents to take pictures and publish the cases, and when they are very young and the representative is not with them, it is difficult for them to accept to have their picture taken and it is totally understandable, we always have to take care of the modesty and vulnerability of the patients.

We also had pregnant patients during the day, but the intriguing part of the work shift were two cases that are rarely seen in the operating room, and these cases caught everyone by surprise.

El primer caso especial que te comentaré es de una paciente con 36 semanas de gestación, un embarazo e alto riesgo por muchos motivos, el principal es por ser un embarazo GEMELAR, también porque uno de los bebes estaba menos formado que su hermano, una paciente con anemia moderada, donde obligatoriamente había que transfundirle sangre, una operación de alto riesgo, tanto para la madre como para sus hijos.

Cumpliendo todo los protocolos necesarios para esta intervención, gracias al esfuerzo del personal de salud y a la Fé de la paciente y los familiares, todo salió espectacularmente bien. Y es que justo en el momento cuando están saliendo del útero de su madre, uno de los hermanos nació enmantillado.

Nacer enmantillado simplemente significa nacer dentro de la bolsa amniótica sin romper, se sigue nadando todavía en el líquido amniótico, este evento maravilloso es algo raro y no es muy habitual vivirlo, también se conoce con los nombres de "Manto de vela", "Velo veneciano", "Parto velado". Se dice que los niños que vienen al mundo de esta forma, traen una carga de dones y buena suerte.

All the necessary protocols for this intervention were complied with, thanks to the efforts of the health personnel and the faith of the patient and her family, everything went wonderfully well. Just as they were coming out of their mother's womb, one of the brothers was born enmantillado.

To be born enmantillado simply means to be born inside the amniotic sac without breaking, it is still swimming in the amniotic fluid, this wonderful event is something rare and it is not very common to live it, it is also known by the names of "Mantle of sail", "Venetian veil", "veiled birth". It is said that children who come into the world in this way, bring a load of gifts and good luck.

El segundo caso también se suele ver muy poco en quirófano y es porque normalmente no se deja avanzar tanto la enfermedad, en este caso estamos hablando de tumores. Tumores que pueden crecer tan grandes que se pueden confundir con embarazos o incluso creer que se esta ganando peso pero por llevar mala alimentación y culpar a la grasa, pero no es así.

Aquí nos encontramos con una paciente de 65 años de edad, que dejó evolucionar un tumor por más de 4 años, este tumor se originó en el ovario derecho. La paciente al principio no sintió cambios ni ningún síntomas de alerta, por eso esta enfermedad paso desapercibida. Al pasar el tiempo empezó a notar cambios en su organismo, se sentía débil, cansada, pálida, pero a pesar de tener esos síntomas no consulto al médico.

Fué hasta a finales del año 2020 que se sintió muy mal con sus síntomas que visitó al médico y él le diagnostico Tumor gigante de ovario derecho, por el tema pandemia la cirugía se vio aplazada hasta estos momentos, llevando consigo unos meses más este monstruo que cada día que pasaba se alimentaba más de ella.

Y es que los tumores a medida que va pasando el tiempo, se va alimentado de todo nuestro nutrientes, porque compartimos con ellos nuestra sangre, es el principal motivo de por que los tumores de este tipo pueden crecer tanto.

También fue una cirugía compleja, por el gran tamaño del tumor, se estuvo que estar pendiente de factores como la presión que ejercía el tumor, la cantidad de sangre que se podía perder, el desequilibrio de la paciente a quitarle una parte de su organismo, son temas que influyen mucho a la hora de la intervención quirúrgica.

The second case is also rarely seen in the operating room and it is because normally the disease is not allowed to advance so much, in this case we are talking about tumors. Tumors that can grow so large that they can be confused with pregnancies or even believe that you are gaining weight but because you have a bad diet and blame the fat, but it is not so.

Here we met a 65 year old patient, who let a tumor evolve for more than 4 years, this tumor originated in the right ovary. At the beginning, the patient did not feel any changes or warning symptoms, so this disease went unnoticed. As time went by she began to notice changes in her body, she felt weak, tired, pale, but despite having these symptoms she did not consult a doctor.

It was until the end of the year 2020 that she felt very bad with her symptoms that she visited the doctor and he diagnosed her with a giant tumor of the right ovary, due to the issue pandemia the surgery was postponed until now, taking with her a few more months this monster that every day that passed was feeding more and more on her.

And the tumors, as time goes by, feed on all our nutrients, because we share our blood with them, this is the main reason why tumors of this type can grow so much.

It was also a complex surgery, due to the large size of the tumor, it was necessary to be aware of factors such as the pressure exerted by the tumor, the amount of blood that could be lost, the imbalance of the patient to remove a part of her body, are issues that greatly influence the surgical intervention.
